On the BMAA Website or somewhere I read that you can make a self made form and get your doctor to sign it to make sure that you haven't any medicals , if you can't do it , then can anyone find me a medical form from the CAA Website (CAA's Website needs sorting out as you can't find anything you are looking for.)
Example:
NPPL Licence Medical Form (This form is signed by the doctor proving that ............ has no medicals or if he/she does have any medicals , then list below and say whether or not these medicals will effect he/she from flying Microlights.

........................................................................................
Signed: .......................................
Something like above would be fine to prove that you have no medicals wouldn't it.
for microlights, the CAA expect you to pass I think level 2 DVLA. if you search the web you will find that form which you can take to your docs to get signed. they will hit you for some cash though. be careful not to get dragged into a full Pilots medical as I did back in 2000!!
the point of the form that the doctor is signing is to say that they have seen your records and that there is nothing in there that prevents you from meeting DVLA level2 driving standard. in fact, the doctor doesn't even need to see you physically!
